An application for services at the Child Study Center is required to be completed prior to a new patient appointment. 

Application Process

1. All requested information must be included with completed application, including insurance information, school forms, and custody agreements.
2. Our triage committee will review your child's application to determine which clinician will best suit the needs of your child.
3. Insurance coverage will be verified.
4. An appointment will be scheduled after insurance requirements for referral/authorization have been satisfied.
5. We will contact you to schedule the appointment and talk with you regarding insurance or any financial concerns you may have.
6. If we are unable to contact you, a letter will be sent to you stating that we are ready to schedule an appointment.
